چکیده

PURPOSE To assess the relationship between apparent accommodation in pseudophakic eyes, multifocal corneal effects, and wavefront aberrations of the cornea. METHODS In 102 eyes of 86 patients who had undergone phacoemulsification and posterior chamber intraocular lens implantation, the amount of apparent accommodation was measured with an accommodometer. The degree of corneal multifocality was determined on the corneal topography by measuring the maximum and minimum corneal refractive powers within the pupillary area. Wavefront aberrations of the cornea were calculated by expanding the height data of the corneal topography into Zernike polynomials for individual pupil size. The influence of higher-order aberration on the retinal image quality was simulated by computing the point-spread function (PSF) and modulation-transfer function (MTF) from the aberration function. RESULTS There was a significant positive correlation between the amount of apparent accommodation and corneal multifocality (Pearson correlation coefficient, r = 0.451, P < 0.001). The coma-like aberration showed a significant positive correlation with the amount of apparent accommodation (r = 0.440, P < 0.001), but the spherical-like aberration did not (r = 0.001, P = 0.993). Among the coma-like aberrations, the component representing vertically asymmetrical distribution of corneal refractive power with greater refraction located in the lower part of the eye was most relevant to apparent accommodation. Computer simulation of PSF and MTF indicated that a focus shift of 0.5 D deteriorated the retinal image significantly more in eyes without higher-order aberrations than in eyes with a moderate amount of coma-like aberrations. CONCLUSIONS Coma-like aberration of the cornea, along with corneal multifocality, contributes to apparent accommodation in pseudophakic eyes.

Apparent accommodation in pseudophakic eyes with refractive against-the-rule, with-the-rule and minimum astigmatism.

AIM To compare the magnitude of apparent accommodation in pseudophakic eyes with refractive against-the-rule (ATR), with-the-rule (WTR) and minimum astigmatism. Factors associated with apparent accommodation were also examined in each group. METHODS In total, 87 pseudophakic eyes (87 patients) that underwent monofocal intraocular lens (IOL) implantation after uneventful cataract surgery were ...

Apparent accommodation in pseudophakic eyes as measured with visually evoked potentials.

We measured apparent accommodation with pattern reversal visually evoked cortical potentials in 27 pseudophakic eyes after the implantation of posterior chamber intraocular lenses. The patients ranged in age from 47-83 years. The amplitude of accommodation was not significantly different between pseudophakic eyes and normal phakic eyes. Apparent accommodation was inversely proportional to the p...

A new intraocular lens design to reduce spherical aberration of pseudophakic eyes.

PURPOSE The aim of this study was to design and evaluate in the laboratory a new intraocular lens (IOL) intended to provide superior ocular optical quality by reducing spherical aberration. METHODS Corneal topography measurements were performed on 71 cataract patients using an Orbscan I. The measured corneal surface shapes were used to determine the wavefront aberration of each cornea. A mode...

Apparent accommodation in pseudophakic eyes after implantation of posterior chamber intraocular lenses.

We measured apparent accommodation in 42 pseudophakic eyes (34 patients) after implantation of posterior chamber intraocular lenses. The mean apparent accommodation was 2.03 +/- 1.03 diopters. The mean accommodative power of 16 phakic eyes used as controls was 2.91 +/- 1.29 diopters. The diameter of the pupil appeared to be the most important factor in apparent accommodation--the smaller the pu...

Predicting the optical performance of eyes implanted with IOLs to correct spherical aberration.

PURPOSE To use powerful modeling techniques for predicting the optical performance of eyes implanted with different types of intraocular lenses (IOLs). This approach will allow performance of "virtual cataract surgery," with different IOL designs that can be used and physical parameters that may occur during actual surgery-in particular, in IOLs that correct spherical aberration. METHODS A co...
